Is my original PDF modified?

No. A new file is produced and the original stays as it is until you choose to replace it yourself.

Can I replace existing text?

To a limited extent. For larger text changes, editing the source document is the more dependable route.

Does a black box remove the text underneath?

Not necessarily — the text can remain in the file and still be extractable. Do not rely on it for confidential details.
